What To Bring

If you are coming for a sleep condition:

  • Medicare card and Health insurance information
  • A referral from your General Practitioner, family doctor or specialist
  • All pertinent reports and test results from your primary care or referring doctor
  • A list of all medications that you are currently taking
  • A list of any known drug allergies and the symptoms you may have from taking these medicines
  • Details of any other treating doctors.

We encourage new patients to come a few minutes earlier to fill paperwork or you may download registration form  and fill it out to bring with you. You can also download an Epworth Sleep Questionnaire to fill out.

If you have a CPAP machine, please bring your machine, mask and hoses for inspection. Please organise to have a CPAP download printed prior to your appointment and give it to reception on arrival.

If you are coming for a respiratory condition:

  • Medicare card and insurance information
  • A referral from your General Practitioner, family doctor or specialist
  • All pertinent reports and test results from your primary care or referring doctor
  • Any x-rays or PET scans or images – bring in your OLD films with your NEW films
  • A list of all medications that you are currently taking
  • A list of any known drug allergies and the symptoms you may have from taking these medicines
  • Details of any other treating doctors.

If you are coming for sarcoidosis, your appointment will last from 1 hour – 90 minutes.

If you need to cancel an appointment:

Telephone the office during business hours and allow at least 1 days’ notice so that we can offer your appointment time to patients on our waiting list.

Confirmed appointments, cancelled at late notice will incur a $120 cancellation fee.

We recognise that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologise if we keep you waiting.
